Registering for your course is easy & secure!
We accept:   check or money order. 1. Click here to download the PDF REGISTRATION FORM. 2. TYPE in your information, then PRINT and SIGN the registration form. Note: FILL IT OUT COMPLETELY as all information is needed to either report your completion to the state or process payment. (If a third party is paying for you, make sure you list their information in the payment section so the credit card number matches the name.) 3. FAX IT TO US at (561) 747-8683. Note: Do not e-mail the form as it has your personal information on it and may not be secure. If you need assistance with registration or payment, we can take your payment over the phone.
